Purchasers must have a valid WI address noted at checkout but may purchase calendars for out of state addresses.

To purchase a calendar, please fill out the information you wish to have entered onto the raffle stub(s) for EACH calendar you are purchasing. A photocopy of the filled out calendar stub(s) will be included with your calendar(s) when they are shipped to you.

After completing this form, hit the SUBMIT button and then add the calendar into your cart and proceed to check out. You can add multiple calendars into your cart at checkout.

2024 Calendar Raffle Entrant Information

Please fill out the information you wish to have entered onto the raffle stub(s) for EACH calendar you are purchasing. A photocopy of the filled out calendar stub(s) will be included with your calendar(s) when they are shipped to you.
